What is GO NFT?
GO NFT is part of the larger GO for Long-Term Care initiative that was introduced in 2004 and 2005 through Gubernatorial Executive Orders and enacted in 2006 through the Independence, Dignity, and Choice in Long-Term Care Act (Chapter 23 of the Public Laws of 2006).
GO NFT is a first step in rebalancing spending for long-term care services by providing a more equitable distribution of public funding between cost-effective home and community-based services and institutionally-based care. GO NFT is an initiative that provides a broad array of supportive services to enable nursing facility residents who are clinically and financially approved for long-term care through Medicaid to return to the community. By providing a consumer-directed package of supportive services, GO NFT maximizes the individual’s independence and dignity in a setting of the person’s choosing.
What services may be provided?
GO NFT participants are eligible for all New Jersey Title XIX Medicaid State Plan services authorized in a plan of care except for Personal Care Assistant and Medicaid Hospice. In addition to State Plan services, the following services are available through GO NFT:
- Assisted Living/Adult Family Care
- Environmental Accessibility Adaptations
- Personal Emergency Response Systems (PERS)
- Home-Delivered Meal Services
- Caregiver/Participant Training
- Special Medical Equipment and Supplies
- Home-Based Supportive Care
- Participant-Employed Providers
Are there limits to GO NFT services?
Yes. The services offered are based on the individual's assessed needs. Lead by the resident, an Interdisciplinary Team that includes his or her family or legal representative, the nursing home Social Worker, the State Community Choice Counselor, the community Care Manager, and other health care professionals, as requested, develops a comprehensive plan of care for the individual. The service package builds on available supports, and takes into consideration the individual's abilities, goals, preferences, risk factors, and the availability of services.
Who will provide these services?
Services may be provided by community agencies or qualified Participant-Employed Providers that can include family members, friends or neighbors. All service providers must demonstrate competence in the service to be provided and must meet qualification requirements, which have been approved by the federal government.
The Participant-Employed Provider service option allows a participant to work collaboratively with a Care Manager to employ a provider and direct his or her own care. The ability to direct one’s own care will be confirmed prior to the approval of a Participant-Employed Provider.
Who is eligible for GO NFT?
GO NFT is for individuals who are clinically and financially approved for nursing home level of care through Medicaid, are residing in a nursing facility when entering the program, are assessed by a State Community Choice Counselor for transition back to the community, and whose planned discharge is coordinated through the efforts of the Interdisciplinary Team. the availability of services.
Is there a cost share for GO NFT?
There is no co-pay for GO NFT except for the Assisted Living or Adult Family Care service. For those services, participants are responsible to pay room and board fees. Those participants with income above the SSI (Supplemental Security Income) amount may have a cost share, based upon their gross monthly income and allowable deductions. The Care Manager calculates the cost share.
How do I qualify for GO NFT?
Nursing facility residents interested in receiving community-based supportive services should ask their nursing facility Social Worker to contact the appropriate Regional Office of Community Choice Options to arrange for a Community Choice Counselor visit.
