Eligibility criteria:

Any patient who meets these four criteria is typically eligible for hospice services:

• Patient has been diagnosed with a progressive, life-limiting illness or condition, and is given a prognosis of 6 months or less to live.
  

• Patient has accepted the terminal diagnosis and decided to stop all preventive and curative treatment.
  

• Patient and the family agree with the goals of hospice care and agree to sign an agreement consenting to hospice services.
  

• Patient is a resident of the hospice providers service area.
